Readygraph app not Working Properly with Simple Subscribe Plugin

Hi I have an issue with ReadyGraph App that comes with Simple Subscribe. My website is http://www.michaellanfield.com
I tried everything to get the app to work on every page and to appear on the bottom right of the screen, clicking on Join the community would pop up in the center of the screen. Instead it only does this on certain pages, and the popup does not appear on the center on the page, instead it appears right above the “Join the Community” which is fine, but the form does not work at all. I am only able to input information, but clicking continue does nothing. When I click continue with Facebook it does seem to work.
So what I tried to do was delete the entire Readygraph App and Simple Subscribe and lost all my subscribers. Damn 🙁 I don’t know what to do. I did export the csv file, but that does not help!
