I would recommend that you watch the tutorials. The videos in the documentation are a bit outdated, but I’ve just recorded new ones for the upcoming 2.0 release:

The first one is kind of long, so if you just want the basics, watch the first 5 minutes of the first video, but I recommend the whole second video (it isn’t quite as long anyway).

I started watching the first one and that was when I began to worry. Do I have to set up all of this stuff or is the default install ok to use?

I watched the second video and I did all that it showed correctly I believe as all my images were optimized. But when I do a Google Page Insights, my score is worse then when I started. It says I still need to optimize images?
I am using wp 3 cache as well. would that be an issue?

flush (empty) your cache, or turn it off entirely and run the test again. I can take a quick look if you want to post your website address.
Regarding the first setup video, the Basic settings are the most important ones:
1. Do you care about image metadata/exif information:
-No, then turn on metadata stripping
-Yes, then don’t
-If you don’t know what it is, then you don’t care about it
2. Are you ok with sacrificing a tiny bit of quality for much better page speed:
-Yes, then turn on the lossy options
-No, leave them off
Note: The tools we use are the best in the industry (that I’ve been able to find). Jpegmini won’t be available unless you have the beta version or wait until next week for the release of 2.0.
Anything else is for advanced users.
