Any answer for this. I have the pdf displaying the fields and I have added a header and footer but I would like to be able to layout the fields similar to how the form looked instead of all fields one after another down the length of the page.
Does anyone have a quick how-to or template to start with for this.

Yes, I have created a custom template but I am stuck with trying to figure out how to get my data to display in borderless tables etc. I was also able to get the data=1 thing to work and I saved all the variables to a text file. I have reviewed the docs but still have not be able to get this to work. I am thinking about sending you a request for a quote. I have four forms created and all are no more than one to one and a half pages when completed. I am sure once I have a template of one of these forms and the code to make it layout I would be able to replicate it for the other forms.

If you are having trouble accessing the $form_data array you can use your form’s merge tags in your PDF template.
