Having Trouble with WordPress.com site

Hello,
I am writing about my site <Mediationand Mindfulness which was my first site on wordpress. I decided to get hostimg about a month a go and now I would of like to know how to switch my content over, but I am not sure is this possible. I have the same followers, but I need them to follow on Lorislittlelessons.com, instead of the old site Mindfulness and Mediation. Please any help will be appreciated. The both sites have the same email as well.
