Issues with page template

I’ve created a simple wordpress theme, and had initially used only the index.php as it is a one page site. I need to create several more pages that have a similar layout, but have different text, images, and forms so I realized I need to create different page templates to accomplish this. I’ve attempted to create a page template here –http://www.fshtnkdesign.com/test, but as you can see it is returning a blank page. I’m wondering if there is a bit of code I’m missing somewhere that is preventing the page template from working properly? Any help, would be greatly appreciated. Thanks!
