sending sms after submitting

First of all of thank you for reading this topic and sorry for my bad english…
im using contact form 7 in 2 page…one for appointment and other for sending question to doctor…

1st form have some field like name,email,phone number,and timethat usese with this plugin :Contact Form 7 Datepicker when some one fill the appointment form and submit it the informations,some fields like name,email and dateو will sms to the person who is filling the form .
the sms text will be some thing like this :

name: (Read from Name field)
email: (Read from email field)
phone number:(Read from phone number field)
Date:(Read from date field)
Thank you for booking appointment

It looks like the confirmation information will be sent by sms instead
of email…but informatino will be sms to the phone number field
