Changing how comments are shown

Hello,

I am trying to get my comments and form to be displayed like this page:

I am using this tutorial to create my own comment form on my theme:

Can someone please help me to figure out how I can get my comments and comments form to show up like this: http://stylista.no/blog/nettenestea/love-warriors#stylista-comments

I am currently running my wordpress on a local MAMP PRO server on my mac. On request, I can send you a .zip-file of my template.
