I’ve seen this happen, but the problem is I can’t replicate it consistently, usually pressing f5 or trying to load PTE again makes the problem go away.
Refreshing the page does not help for me.
Do you see anything on the javascript console? if you open the network tab are any of the calls red?
Does this always happen to the same images? Are those images a bigger file either in file size or image size (width x height)?
Is your server running linux or windows?
I also have this bug on several sites with this plugin.
As mentionen u have to push F5 again and again untill it works.
It feels like there is a problem with javascript code running before the image has loaded – just guessing…
But i´m waiting for an update with a sollution for this.

@turbojohan I’ve been having the exact same problem. Originally I was getting a 404 message from the CSS trying to load one of the FontAwesome files. So I removed the declarations for that from the CSS and still having problems.
@sewpafly there’s no obvious reason as to why this is happening. I’m using whatever the latest version of Chrome is at the time of writing this reply. But I’ve also tested in IE 11.
What appears to be a workaround (or at least works for me because refreshing only worked a couple of times, bespite 20+ refreshes of the page) is to simply click to another admin page then use the browser back button. So far, this has loaded the preview image from where you can then crop every time.

Yes it works and I have tested it in Chrome och Firefox.
But this is still a very temporary sollution.
I hope the developers will solve this soon.
@turbojohan and others – if you’re still having problems, try the latest PTE version and enable the save to disk option (Under crop picture size (http://sewpafly.github.io/post-thumbnail-editor/editor/#user-options)), to see if this solves your problem.
