Displaying Extended Profile Fields on Member registration and Admin

I am not using word press default registration page.
But I want to add some extra fields using this plugin on my website(emedsolution.com)registration page(http://emedsolution.com/join-us/). But I am not able to insert these extra field there. Can you please guide/help.?
