Same for me. It’s blank. Please help!
I am having the same issue. I am trying to add a form lightbox to a post but when I click on the “Add Form Lighbox” button in the visual editor the popup is blank.
Exact same problem … all options missing and only see a blank screen … oh dear!!
was this issue resolved. As I am also having this problem.

Me too – I’d love to try this plugin. But the add-form interface is a blank window.

I had to downgrade to Form Lightbox Version 2.0.1, which works with Fancybox …

Working well for me in conjunction with Gravity Forms. Everything breaks, however, when I go to Form Lightbox 2.1.
