One users avatar disappeared.

A couple months ago, one of my BuddyPress members avatar was something she didn’t want, and no matter what we did, she couldn’t change it. She could upload a new one, but it wouldn’t change. The only way we could fix it was to delete her user, and start a new one which worked fine.

Then this week, my avatar disappeared. It now shows as a broken box and I can’t replace it. I’ve tried deleting and re-associating the gravatar, uploading a new avatar on the user profile page, and ran fix db on the database. In both cases only one avatar was effected.

How can I fix this?
