Can't Get it to Work

I am trying to enable this plugin on WordPress MU 3.41

It doesn’t turn on. I keep seeing the message “NOTE: Pugin is not active yet. Please set Enable from Enable Hotlink Protection option!”

Checked the error logs and I saw this:
