Complex Form migration.

Hi, We have a site under dev. and it is almost ready to launch. the only thing we are waiting on is to get a patient registration form working.

here’s the deal. The form was working until we changed themes. I copied all the dependency files I could think of and it appears to launch ok but it does not show the REQUIRED filed asterisk.

I cannot figure out what to do.
