Viewing new field after checking checkbox

    Hello,

    Here a some more complicated i have a smart contact form like:

    Name / Last Name
    Mail / Phone
    Select Product [3 three options]

    checkbox [I want to recive a invoice]

    What i mean -> When check this option “I want to recive an invoice” after check this i want to open somee extra fields for fill like:

    company name
    IDN:
    Adress
    City

    But only it should show after checking this check box – can u help me??

    1 more thing…

    have a checkbox like “I accept term of use and privacy policy”

    when try input url for term of use and pricy policy this fild going hide 🙁 is possible to make a url adress in checkbox label?
