Page for client to submit updates

Hey Everybody,
I’ve created a password protected page I’d like to send users to in order to update their account with my company. Specifically, we manage their phone IVR’s so I’d like them to log in and see a page that shows 1-9 and what each option is for. Then, I want them to be able to click, for example, number 4 and change it from “Bob” to “Phil”. I’d also like it to notify me when the changes are made so that I can log into our system and make the actual change. Any suggestions would be greatly appreciated.
