User Feedback with rating widget

    Hi,
    I want a widget which will let the user put some comment and star rating for that page. I should be able to add that widget in sidebar.
    “Post a comment” functionality is must.
    I tried for different plugins but not getting what i want.
    For Ex: I am going to use this widget for a Person.
    i want to rate that person.
    I should be able to add a review about that person.
