Website / Date not showing up

Hi there.

I have added your plugin into a site I am building here: http://www.southporthosting.co.uk/g2s/testimonials – but the testimonial date and websites are not showing… can you please advise?

Also, I think your plug-in is great, but could you advise how I remove one or more of the fields from the add testimonial section – so if I wanted to remove the option to add a website address for example.
