You may have missed a step in there, so check it carefully.
What version of AWPCP are you running here?
Also, can you post a screenshot of your Classifieds->Settings, Form Fields tab to show me what fields you have displayed?
Finally, please post a screenshot of your Classifieds->Categories section, so I can see what categories you have enabled.

But to be honest, I would like to figure out these 2 problems as mentioned in the first message:

Looking at those links again, they sure look like some kind of plugin or theme conflict is going on–when a form is completely missing like that on Place Ad, it’s 99% likely that the theme is doing something, or the plugins are conflicting. For the second problem (the contact form), there is no “contact form” on the ad itself. You must click the “Contact [name]” link to see the form. It’s not embedded in the ad. I think that’s a misunderstanding.
Clicking on the Contact Peter… link shows a form. So the reply to functionality is working, although it’s styling rather strangely.
For your category problem, it appears to be some sort of theme conflict, as I suspected.
When I switch to TwentyTwelve, I see the Place Ad form, as I expect. But with your Green Chilli theme, the page is wrong.
For something like that, it’s most likely one of the following:
1) a jQuery conflict (theme has an older version than AWPCP)
2) Unclosed or invalid HTML in the theme
The first is easy to change if you are comfortable modifying PHP files. The second you can check their page content with an HTML validator tool to find the issue. But in both cases, the problem is in the theme, not AWPCP unfortunately.
