This message could be from a lot of things. I’d be surprised if it is from the link you referenced. Best thing to do is fill out my Help and Support form and I’ll try and replicate the problem using your form on my site.

Looks like the WordPress.org site had a hiccup – no idea why my response was posted so many times!

Is the problem you’re having new? What version of the Google Forms are you running? Have you changed anything else on your site?
