Register to New Database Per Form

Hi,

I’ve searched through a few different plugins but can’t find anything I’m looking for, and am wondering if anyone here would know of anything off the top of their head.

I’m utilizing wordpress for a cluster of radio stations and we need a plugin that does the following:

Allows us to create a registration form, that, when people signup, it simply goes into a list or database in the backend that we can select a winner from at the end of the contest.

Then, the next time I make a new contest/form, it’s a separate list from the previous.

Does that make sense? Not sure if this exists, but if someone could give me some direction that would be great! Thank you so much!
