    Form appears to be displaying correctly in Firefox 21.0.

    Please describe your problem in more detail. If others here are able to understand your actual problem, they may be able and willing to offer a possible solution.

    I am using Chrome. Apparently, it took a few hours for it show up because now it is working and the form is displaying properly.

    I do not understand why it took so long for the form to finally show.
