Display form without using shortcode

    To clarify a bit, the page I’m working on is http://www.setartistmgmt.com/v1/contact/.

    The tabs on that page are created by shortcode provided by the premium theme.

    I tried loading the form in page without other shortcodes and it works, so I took the HTML (via the debugger). When I place it in the tab, it partially works (validation doesn’t work).

    Can you help?
