That is among the features that are next on the TODO list. The only workaround would be to edit the code of the plugin to send an email to the admin email address of that site.

I have added a plain email notification for uploads to the Development Version which currently notifies the admin email address that a file has been uploaded. Since it is a development version, I have not yet added settings to control the notification. I am still working on the shortcode aspect and should have something soon.

This would be top of my wish list too.
Using the plugin for a Scouts web site and have to send contributors to another form to enter these details.

I would love a comments and contact field also. I have two use cases to consider.
1) A church website where people could upload their own pictures of events. A comment field where they could give the site admin a description and their contact info would be helpful.
2) Similar: A local news site relies heavily on community reporting. This is currently done via email. A way to give a caption, a brief description, and contact details would be awesome.
The custom field feature is currently in development and I hope to have its released for testing soon. I will post an update here when I have gotten to a point where I need a few people to test it out. With the upcoming release of WordPress 3.6 and the changes they are making, I am essentially programming for two different platforms hence why its taking so long.
