Customizing date field

Hello,
I’m working on a website and made a special design for the dates.

That is the design. Now, i have no clue on how to start with building this. I want this to be automatically genreated when writing a new blogpost.

I also want all the months to only have the first 3 letters (in Dutch) to show up.

Can anyone help me out with this? Thanks.
