The form has suddenly stopped showing on the page and I have no idea why. I’ve upgraded to the latest version, removed the form, created new ones, added new shortcodes, nothing works.

    Here’s the page in question:
    http://thebutchclothingcompany.co.uk/contact/

and on line 152 of the source you can see where the form SHOULD appear. And the form is set up as:

    <p><strong>Your Name (required)</strong>
        [text* your-name] </p>

    <p><strong>Your Email (required)</strong>
        [email* your-email] </p>

    <p><strong>Subject</strong>
        [text your-subject] </p>

    <p><strong>Your Message</strong>
        [textarea your-message] </p>

    <p>[submit "Send"]</p>

[Please post code & markup between backticks or use the code button. Your posted code may now have been permanently damaged by the forum’s parser.]

Can anyone help, or suggest why this isn’t showing?

Any help or advice would be much appreciated. Thanks!

You are using Contact Form 7 3.1.1. The latest version is 3.3.3. Update it first.

Very odd. I updated to 3.3.3 already.

I’ve removed the plugin, and installed again, but still showing 3.1.1 on the page source.

Maybe there’s something in the DB that’s making it not show?
Something maybe went wrong in a previous upgrade (the form stopped showing a couple of weeks ago, around the time I last upgraded).

What’s the easiest way to flush all the data for contact form 7 out of the db and start fresh?

The DB is not relating versions. How did you install the plugin?
