Testimonial Field doesn't show

Hi,
I’m running 3.5.1 and installed version 1.3 of GC Testimonials…
When I add the short code for the submission form, all the input fields show up except for “Testimonial”.
How does one fix this issue?
I’ve attached a screenshot of what I see when i visit the page with the submission form.
