CAPTCHA code – Cookie error

    I have several forms that have required fields including attachments, but the CAPTCHA code seems to be an issue. Upon submitting a form, an error code appears saying something about a “cookie error”. The CAPTCHA error then clears any attached files, which has to be re-submitted.

    How do I fix the CAPTCHA code error?
