Jet pack subscribers problem

I am trying to set up my blog so that users can have an email sent to them to let them know when I have posted a new post and possibly be able to read the first paragraph of the post in the email. I have wordpress 3.5.1 installed and jetpack 2.1 – I have the JetPack Blog Subscriptions put into the Jetpack In Page section in the Widgets. However I am using my own theme integrated with my website…..and I am not seeing the form. Where can I get the form code so I can add it in myself?
