Multi-Language with Domains

Hi,
I have 2 domains, say: http://www.sample.de and http://www.sample.com. Now I have WordPress running with a nice Theme.
Now what I want to achieve, but dont know how or which plugin to use:
1. I want my site to have 2 Languages: English and German. I want to write both languages for each Post/Page/Portfolio-Item myself, so no automatic translation or agency.
2. I want to have the language choice in the header (small flags or just links if I want en or de)I saw some plugins, which handle the first 2 points, but now to what is giving me headaches, and Im really lost without you awesome guys here:
3. Although I want to have the choise on every site to swich the language, I want the domain to set the initial language, so:
If I go to http://www.sample.com, I get the english language version.
If I go to http://www.sample.de, I get the german language version.Not only that, I really would love to have nice urls, so:
If I would be on Portfolio page 3 in english, the url bar would say: http://www.sample.com/superniceproduct3
If I would be on the same site in german, the url should be:
http://www.sample.de/superniceproduct3Other example: Contact formular should be in english:
http://www.sample.com/contact
in german: http://www.sample.de/contact.What I really don’t want:
Language shortcuts behind domain: http://www.sample.com/us_en/What I really don’t want either:
If a en page(or else) is on http://www.sample.com/page2
and I go to http://www.sample.de and click on the link to the site, that it swiches the url to http://www.sample.com/…Heres what I can do:
My server has new php and mysql installed, so all newer plugins should be no problem.
Also I can redirect my urls to specific folders. That is acctually happening now allready: The site is on http://www.server.net/blog and both sample.com and sample.de are redirecting to that folder.
So if it would be possible to direct sample.com to http://www.server.net/blog/us_en and the sample.de to http://www.server.net/blog/de_de and everything would still run, that would be also great…Ok, I wanted to make it short, but the post turned out rather long. But hopefully somebody can help me or direct me towards the right plugins and If so I will report that everything worked and hopefully this will help other people too. Because I think this is a rather common problem I’m trying to solve here.
