Add Additional Fields to Widget

I have installed the Widget which works great.

When people donate to my cause I would like to give them the option of filling in a few fields such as “Donor Name”, “City”, ect.

Is there a way to add these fields to the button so that the info can be passed on to Paypal when they make their donations?
