Cannot Add Channels after 3.1 update

We have been using this plugin for months now and have had no problems. We logged in today to see there was a 3.1 update so we updated. Now when we go to add a channel it will not display the input boxes like it used to.

Is there a link to the previous version that worked for us or do you know what is wrong with it?
