More on this… I was just playing around, trying to create more forms to see if I could learn anything. Apparently previous forms disappear when I click “Save”.
One MORE thing: I’m using WP 3.0.4, not 3.1.4 as previously stated.

about the form saving issue: yes, you are the first person to report such a problem. Jazzy Forms has never been tested with WP3.0.4, but this is not necessarily the answer to this issue. It might also have to do with the browser you are using. Which one is it, please?
about 1 (number formatting in emails):
In order to maintain an Ouputput element’s number formatting you can use the FORMATTED function. For example, change your placeholder from {{x}} to {{FORMATTED(x)}}
about 2: (radio buttons without defaults)
Do you think so? Although browsers support radio buttons without default selections, it is rarely seen on well-designed online web forms. It’s kind of contrary to the real-world radio metaphor that is behind this element, isn’t it?
about 3: (links in emails)
I’m glad you like this, but I haven’t done anything to achieve this! I reckon it is your email client that converts URLs to clickable links automatically.

I am using Firefox 15. I just tried IE 8 to see if there was a difference and, interestingly, my form is blank (all the elements are no longer on the right side) and when I click “New”, I am taken to the Dashboard in WordPress (??).

2) I just thought it would be easier for me. I had first selected default options and then decided I didn’t want them so I had to delete and re-create values. Not a big deal, but a tad more time-consuming.
