plugin for custom forms – can any do this?

Hi all

Okay, I’m working on a site for a client.
They really wanna put their order forms up on their site as they ‘used to be’… and have a neat output of the things submitted..
I’m not able to explain briefly the layout and function, so instead I turned to photoshop and did this:

I’m willing to pay for a plugin (like gravity forms) to do this. I just want a relatively simple way to set it up – one that I can maybe learn my client. Drag ‘n’ drop like things are totally preferred, since they are very new at webstuff like this 🙂

Anyone having any suggestions?
