[Plugin: Buddypress Sitewide activity Widget] Admin widget options are MISSING

When editing widget options, the last two options arent showing any of the checkboxes.

I have tried disabling all plugins, deleting and reinstalling the plugin, and trying in my other blogs (which all worked perfectly), not sure why it would be different here.

Can I program my choices into the code?

Any other suggestions?
