Form Integration

    Hi

    Wondering if someone can help? I am willing to pay….

    I need to integrate a form into a page on my wordpress site. The form is a domain lookup form.

    The form I would like on one of my pages is similar to the one at http://clients.puretigerhosting.com (this is my site however that sub domain is different software WHMCS). When I use the integration from provided by WHMCS I get the following….

    http://puretigerhosting.com/domain-names/price-list/

    What I would like is one input field and one button that says ‘check’ all on one line.

    Can anyone help?
