I switched plugins to another contact form and everything works great and I get excellent support. It was just easier to do it this way because this particular plugin was not going to work with other plugins I use.
Just FYI I have a site solely dedicated to checking for conflicts with plugins I use. This has helped me a lot.

I re-installed the plugin from the wordpress control panel and now it works fine.
I’m having same problem as first post on this subject… when adding any Form Element to my new form, a circulating icon appears to the right of the Select Element box and doesn’t go away. Latest WP installed 3.4.1. I have no other contact plugins installed. Thanks.

I would give access to the XYZ SCripts Team and they should be able to help you. As I said before, I switched plugins and my contact form works fine but what works for me may not work for you.
I have a test site where I test all my plugins and themes first. It is particularly helpful when upgrades start. Rather than bring down the main website, I don’t mind if the test site crashes. And I don’t stress out if a plugin is not working.
