    Can anybody point me in the direction of a nice, free order form?

    Here is exactly what I’m trying to mimic:

    eyefatigue.com/orderonline1.asp

    Upon submission it is to be redirected to Paypal.

    I’ve never done an order form before, so I’m quite lost.

    Any help would be appreciated!
