[Plugin: Custom Permalinks] No luck

Downloaded this hoping it would do this, but not sure if I’m barking up the wrong tree.

I simple want to change my permalinks so my pages at:
http://www.evolutionglass.com

show up as evolutionglass.com/name-of-the-page (e.g. service, contact, links, etc.) rather than the random page numbers. Can anyone tell me the best way to do that with this?
