This has to be set to a location rather than determined from the user’s IP? Not sure I understand what an arbitrary starting location for determining distance does for the user. Am I missing something?

Most people don’t have national or global coverage so IP / HTML geolocation doesn’t make sense for everyone. With that being said I just pushed 2.4.4. There are a ton of updates here as it jumped from 2.2.5. See http://simplemap-plugin.com/changelog for details. Auto location is included.
