How to make "report" function that reports a post

Hello.

I have a wordpress blog with embedded videos in my posts. I need a report function that reports the post id og url – it doesn’t matter, I just need to know which post is being reportet. The id og data doesn’t have to show up fancy or anything. I tried looking through the plugins section, but didnt find anything. Do u guys know if its possible?
Im looking for something extremely simple 🙂
