    This could be related to a permissions problem on the custom-contact-forms folder in WP themes.

    I was having the same problem, and checked the permissions of the folder was 755. I used filezilla to change the permissions to 777, and now the attachments are sending.

    I’m not trying to send attachments. Notice I said attachment fields in the back end to setup the form.

    I had the same problem and found that it interfered with some of the other plugins I had installed. Deactivate the other plugins and activate them one at a time and you may find one or more that break the Custom Contact Forms admin page.

    I wound up switching to Gravity Forms.
