Javascript inside a page?

I’m trying to implement a drop-down HTML list in a simple page so that when an item is selected a JS script is executed. Here is the HTML of the page:

The functions setup() and lookupState() are defined in the header.php template for my theme as recommended by http://codex.wordpress.org/Using_Javascript
