Canvas size selection app problem

Hey everyone. Recently a client of mine who owns a print canvas company asked me to install a “canvas order form” for him on one of his sites. Here is a link of the working app on another one of his websites:

As you can see, you can select the size canvas you want to order by selecting the price with a radio button.

He asked me to install that same app on his other website. I had to create a page separate from wordpress so the left hand side navigation bar could be removed for this one page so the app would fit on it. Here:

As you can see, there are no prices for the width/height of the canvas’s. I can’t seem to find where these are stored. I looked in the database, there was nothing. Looked in all the files, couldn’t find anything other than a file with an equation that totals up each width/height.

Has anyone ever used this app and know how to do this? Thanks!
