[Plugin: WordPress Form Manager] Custom Display the form

Hello hoffcamp,
First of all, thank you for the great plugin.
I have question is that I want to customize a form without tables. The table tag interrupt my web design. Where should I look for the table tag to remove?
I think the table populate in displayFormBare function on display.php and I removed

,<table>,<tr> and <td>. However, table and ul still showing up.
Please tell me where to remove

and <table> tags for shortcode form?
