[Plugin: Contact Form WordPress] Almost works

I really wanted this form to work. It has everything I need and is simple to use. However, it doesn’t let me add more than one field. I click on the green “plus” sign and nothing happens. Tried deactivating and reactivating, uninstalling and reinstalling – nothing works. It does let me delete the field which is strange…
