If you have any other plugins installed, try temporarily deactivating them all, just to see if there is a conflict. Most of the time this is the case. Then let me know which plugin(s) cause the conflict, and I’ll try to fix it.
The conflict was with the keyword statistics plugin.

Okay, the keyword statistics plugin is inserting some javascript into the Form Manager sections and this is causing the save script to break. Unfortunately there is nothing I can do to fix this on my end, you will have to ask the author of the keyword stats plugin to only add the scripts to the appropriate sections.
The javascript is added to the footer of the admin pages, and it is crashing when looking for the ‘kslang’ element, which only exists within certain parts of WordPress.
