TDO Mini Form – Customize Form

I recently upload the TDO Mini Form Plugin, but when I created the form it looks all messed up. I tried customizing the form but can’t seem to get any changes done. Please help me, I would like for the form to look organized, sort of like the first screenshot http://wordpress.org/extend/plugins/tdo-mini-forms/screenshots/ Any help is appreciated. Thank you.
