User Reported Snow Pack Conditions

I am looking for an addon that allows for categories such as XX Road, which allows registered users to supply current conditions on that Road. I don’t want something that pulls from another sites weather data as this is for people reporting conditions on the front line. It would need to support location (category), a blurb about conditions, reporting party/user, and a time stamp allowing for chronological order.
The main page of this would then need to support all recent posts from all categories (say 20 posts), and have a unique appearance for moderator posts if possible.

Does anybody know of an addon that would support this functionaliy?
