How to create custom "comment" page

Hi,

I would like to be able to use some kind of layout for the comment section.

So, when you like to comment. Instead of 1 big box. You see additional details.
Name:
Age:
Income:
Etc:

And…These details should remain where they are once the comment has been posted.

So the posted comment should look like this:

Name: xxxxxxxxxxxxxxx
Age: xxxxxxxxxxxxxxx
Income: $1000 000
Etc: xxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxx

Is there any way to achieve this ?
I have been trying it with plugins and tutotials, but no succes.
