Email and Social Subscription Widget

Hello WP,

I am in search of a plugin, if some one could please guide me. The widget or plugin should be exactly the same as i explain below.

I want to have one box Title. eg.: email and social subscription.
–an icon or image of a post followed by a blank text box (text box for user to submit there email) and then a subscribe button.
–below this there should be twitter, facebook etc buttons for subscription.

I hope i am able to clarify what i want.
There was a blogger, who has the same setup, he helped me but that needs a little bit of programming and i am zero with it.
